# Collect and verify a W-9

Collect a W-9 form, sign it with test data, and confirm that Avalara stores it correctly.

Before you begin

Make sure you can access the Avalara UI and the W-9 form.

About this task

Use this task to complete a W-9 with test data and verify that Avalara records the signed form.

Steps

1.  Select Fill out W-9 in your web browser.
2.  Enter sample information in the form, then select Sign & Close.
3.  Verify that the page updates with the date and time the form was signed.
4.  In the Avalara 1099 & W-9 application, select your test company from the list of companies.
5.  Confirm that the completed form appears in the list of forms.

    Note

    A `form_request` can be used only once. If you reload the page or return later, you won't be able to complete the same form again. To submit another form, create a new `form_request` and update the `w9-example.html` file.

Result

The W-9 is signed with test data and stored in Avalara. You can confirm the signed timestamp and form availability.
